To file a notice, email our designated agent with: (1) your signature; (2) identification of the copyrighted work; (3) the URL of the material on OnlySpotlight; (4) your contact information; (5) a good-faith-belief statement; and (6) a statement, under penalty of perjury, that the information is accurate and you are authorized to act.
